If you refuse to accept government-imposed health insurance, you will be fined $1900. If you refuse to pay, you can be charged with a misdemeanor, fined up to $25,000 and jailed up to a year.

Ensign receives handwritten confirmation
This doesn't happen often enough.
Sen. John Ensign (R-Nev.) received a handwritten note Thursday from Joint Committee on Taxation Chief of Staff Tom Barthold confirming the penalty for failing to pay the up to $1,900 fee for not buying health insurance.
Violators could be charged with a misdemeanor and could face up to a year in jail or a $25,000 penalty, Barthold wrote on JCT letterhead. He signed it "Sincerely, Thomas A. Barthold."
The note was a follow-up to Ensign's questioning at the markup.

It seems pretty clear to me. They want to enforce mandatory health insurance by fines and imprisonment using the IRS and tax codes.

The precedent has been set by the States. Mandatory auto insurance. They've just kicked it up a notch or three.
